where can i print the FMLA form?
I spent nearly an hour on the dept of labor website, and on the dept of education website trying to print out this FMLA form. Anyone know how I can find it?
I stopped working already so my payroll secretary can't just give me one, and God forbid she helps me out and faxes or emails it to me... Thanks in advance !

The FMLA form should be provided to you by your employer. I don't believe there is an"official" one from the DOL. (I am a former HR Benefits Manager and I always used a company specific form that we created).
Contact the HR Department at your job - they are required to provide you with the info if you are entitled to FMLA.
