When to get a 2nd opinion with reflux
My DD spits up out of her nose often. It definitley hurts her and she cries and stiffens up, arches, turns red, foams at the mouth with spit and her eyes bug out. It's really terrible to watch. I pick her up and console her but she is still in pain.
She is on axid for pain but the dr doesnt think she has bad reflux becasue she isnt colicky cranky all the time..just when she spits up out of her nose. So he didnt put her on zantac becaus he didnt think it will do anything. I think she also has a milk protein issue since she stopped arching and crying while eating as soon as we switched her to aliumentum.
I told the dr she was still spitting up out of her nose and he had us try cereal. I tried for a few days and didnt see an improvement so I stopped. Plus she was spitting up whole bottles since I had to use a level 4 nipple for the oatmeal to get through and it came out too fast (even with pacing her)
So my question is, is this normal or should I ask the dr for zantac or get a 2nd opinion. This dr is loved by a lot of ppl who see him but I kinda feel like we were just shoved to the side and there is no feedback or anything. Do I go back to him? After the upper GI came back fine I called to see what the next step was and it took them a week and several calls to get back to me to tell me results and tell me to give her oatmeal.
I feel like I should be doing more since she is in pain with this. Or is this just something she has to deal with because she has reflux or whatever the hell she has.

She probably does have a milk protein alergy. Demand that your Dr test a diaper for blood. Simple. If he wont find a new Dr? I'll never understand why a Dr would choose an unhappy baby and stressed Mommy when all they have to do is test some poop!

They did test and there were traces. We already started the aliumentum so it may have lessened. He didnt think there would be any since her symptems were not too bad.

Re: When to get a 2nd opinion with reflux
Take her to a pediatric GI.... My dr was good with my son and his reflux and put him on Zantac right away... And he kept on top of it. The dosage is very weight sensitive. My son was diagnosed at about 9 days old with a severe milk protein.soy allergy. We consulted a pedi GI right after that..We changed formula 5 times before he was ok with it.

When to get a 2nd opinion with reflux
I went through the SAME EXACT THING! I just posted in your other thread. From about 4 weeks old my DD started having these crying spells after spitting up. My Ped put her on Axid. It helped, but we would have to keep increasing the dose every 2 weeks with weight gain. Then we started having episodes where the milk came out of her nose, her eyes would bulge, she would turn red, gasp for her breath and flail her arms. They started out happening maybe once every 2 weeks, we would increase meds episode would stop then BAM it would happen again. At some point the Axid stopped working. The episodes increased to every few days and she was just worse and worse. Explosive diarrhea, really painful bad gas, crying spells for an hour, just all around horrible. Mind you she was still a happy baby, she would just have these episodes every few days and I just was at my wits end. The whole time I kept asking my Ped why dont we change her formula, I feel its her formula. My ped is a VERY well respected ped and I do love him and I do trust him, BUT this 1 time I went based on my instinct. From Axid we went to Prevacid (she started throwing that up) then to another one I cant remember which one that was, also bad reaction to it throwing it right up) then Nexium which was WAY too strong for her and changed immediately to Zantac. At the same time I decided it was time to change her formula to Alimentum. Well I got a WHOLE new baby. She is happy, she has normal bowel movements, shes a happy spitter when she spits up.
The coming out of her nose thing unfortunately you may have to wait until she outgrows it. My DD just kind of stopped at some point. (knock on wood) I tried the oatmeal and then the rice and I had the SAME EXACT issue as you. I couldnt find a nipple that worked and she would just throw up the entire bottle so I stopped. My DD is now 6 months and has been on Alimentum for about 2-3 months now. She is doing EXCELLENT! Just stick with the Alimentum and I would personally demand Zantac. I found that it helped my DD along with the Alimentum. I went to a GI specialist and an ENT specialist. My pediatrician was afraid to change her formula because the Alimentum is thinner and therefore he felt it would DEF come up more often, but I didnt listen to him. I changed her formula on my own because I knew it in my gut THAT was the culprit. And sure enough it was. Do some research and you will see milk intolerance can mimic all the signs of reflux and in fact exacerbate it. They were ready to do all kinds of testing on my poor DD, even sent her for a sono to ensure her digestive tract was working properly which cost me $250! Meanwhile it was her formula all along. Sometimes Drs arent always right. Demand the zantac.
ETA: Please FM for more info. I did SO much research on this I would be happy to share.

Re: When to get a 2nd opinion with reflux
my girls were both on axid for MONTHS. THey both spit up out of their nose all the time and were so uncomfortable. They did out grow it coming out of their nose. THat was when we saw if the axid was working or not. It was, but they just needed to be a little older. I dont remember how old they were, but it got to a point when they would start crying we would realize that we forgot to give them their meds.
I also agree with the above poster that VanCura is awesome!!!

Re: When to get a 2nd opinion with reflux
My DD also didn't spit up often, but when she did, it came out of her nose. Even recently, between 5 and 6 months, she has had 4 vomiting episodes while eating, and each time, it came through her nose. She was on Axid at first, but then changed to Prevacid, which helped a lot. Even though it's just supposed to take away the pain, not the vomiting, the only times she threw up through the nose was during the period of time when we were trying to wean her off of it (as per her ped), by cutting out her evening dose. We started giving her the evening dose again, and no more vomiting. Could be a coincidence, but my guy says no.
Also, my DD continued to test positive for blood in her stool while on Nutramigen, and had to be switched to Neocate. She still has eating issues, but at least we've eliminated those two issues.
ETA: I also recommend seeing a pediatric GI. We use Dr. David Gold at Good Sam.

We see bilder mostly. But we see gold for Gastro. They are the ones who I had trouble with calling me back.
Eta- they apologized but it left a bad taste for me. And he said the acid meds won't do anything for her since she isn't uncomfortable all the time.

Re: When to get a 2nd opinion with reflux
DS has a milk protein allergy and reflux...it was a rough first few months..but once he went on alimentum at 4 months it got much better.. I will tell you that Zantac was terrible for him..the minute he went on it he was grunting..and very uncomfortable..and he could not tolerate it..we then went on pepcid and that was great for him.
Follow your gut, if you are not satisfied, go to a GI..or see another ped in practice, if there is one.
Good luck..

When to get a 2nd opinion with reflux
I go to the same practice. I'd try Dr. VanCura. She helped me immensely through DD's reflux issues. I like Dr. Builder too, and we saw her quite a bit in the beginning because of the Sunday newborn hours, but honestly every time I asked her about meds for the reflux she just told me it wouldn't cure it. WTH???

Why didnt I know about these Sunday newborn hours?? I see her during the week when all the sickly kids are there. :(
I saw a GI already. He's the one I want a second opinion on meds from. (That doesn't make sense. Lol. It's late)

I know that Dr. Gold tends to think that reflux meds are often unnecessary. DD's were originally prescribed by her pediatrician, and Dr. Gold said that they are probably overkill, and that most discomfort is due to trapped gas. I don't necessarily agree with him, and have also been debating going for a second opinion regarding DD's feeding resistance.
I have not had problems with him calling me back though. He always returns my call after 5 or 6, except once on Wednesday, because that is his hospital day and he doesn't always return to the office.

I sensed that about him. but its like cant we try? Her main issue is she spits up out of her nose. Its not alot but out of the nose means its forceful. He said projectile spitup is forceful too and is a sign of reflux but she doesnt really have that so he said its not reflux. Although she does a bit when she spits up her entire bottle after feeding. I think thats more of a gagging on a burp thing though.

The sunday hours are GREAT!!!!
I had a bad experience with Dr. gold as well. I was not happy there at all. His only solution was for my daughters to switch from BM to formula. I told him I was not interested in that and he literally put the formula in the stroller. In the end I called my LC back and we created a new eating plan for me and it did help. THe girls were about 3 or 4 months adjusted then the axid was also working at that point.

I'm surprised the office didn't tell you. Sundays are great, especially at this time of year. You can bring them on Sundays through their 5 month appt. Builder is the only doc that works those hours I believe. At least she was when we were going.

He did say that there is a super hypoalergenic formula that we could try if she got worse (most likely Elecare). He said usually it gets bettr then gets worse. It got better but she still has the nose spit up. Our insurance excludes all formula no matter the reason. The employer dictates what each plan covers and DHs doesnt cover. Mine covers formula but its over $600 per month to add a child/children so its not worth it. Right now the aliumentum is $300/mo at the amount she currently drinks.
All spitting up is reflux but he said she doesnt have an acid problem or she would be crying all the time and she doesnt. She doesnt cry when she spits up, just if its out of her nose.

To me that rationale is ridiculous. Well she vomits out of her nose regularly-but it doesnt hurt her so no worries! I would worry about her swallowing or inhaling that. I don't know, I would absolutely try the elecare-though I'll tell you my son would not touch Elecare but loved Nutramigen AA. Its also easier to get samples of Nutramigen AA

This is why I want a second opinion and I'm trying to see if its normal for her to spit up and have it hurt. Like if its just par for the course for babies with reflux.
Is the AA different from the nutragamen that is the milk protein allergy formula that's equivilant to aliumentum?
I switched my regular ped apt to a dr who people say deals more with reflux and ill make an apt with the Gastro and give him one more shot.

Yes AA is amino acid. Sorry to be so pushy, but my son suffered with this for 9 weeks until I finally forced my Dr to give me something!
