I had this happen once during early pregnancy. I asked the Dr. about it and she said it was probably an internal hemheroid. Unfortunately, there's not much you can do about it. You may want to ask your Dr. about a stool softener to make it easier to push. A nurse friend of mine also suggested putting a little Vaseline down there when you're ready to go so it can help things slide it. I know, it all sounds pretty gross, but whatever works!
