Teachers, do you have your maternity leave template?

I dont know if it is pregnancy brain or what... but I dont know where to begin my letter.

Does anyone have a template I could use?

I plan on taking the 12 week FMLA.

8 weeks - c section recovery using sick days and the last 4 weeks unpaid.

Dated Sept 29th:
This letter to formally let you know that I wish to take Maternity and Child Care Leave during the remainder of the 2009-2010 school year. My estimated due date is October 15th. Upon delivery, I wish to begin paid Maternity Leave for as long as my doctor recommends I need time to recuperate. I will follow up with a letter from my doctor after the delivery. Beyond that, I am requesting to continue my leave as Child Care Leave through the end of the 2009-2010 school year. I look forward to returning to xxx School in September 2010.

After giving birth:
I am happy to announce that I gave birth to my son, Kenneth Thomas, on October 19, 2009. I am enclosing a letter from my doctor, Dr. S, regarding my recuperation period from caesarian section following the birth of my son. He recommends that I recuperate for a minimum of 12 weeks until January 11, 2010.

After my recuperation period, I wish to continue my leave as Child Care Leave through the end of the 2009-2010 school year. I look forward to returning to my position as a special education teacher at xxx School in September 2010. Please let me know if you require anything further. Enjoy the remainder of the school year.

You're welcome! My OB just hand wrote the recoup period on an Rx form and I attached it to the letter.
