Stupid Question-how do you switch peds?

I mean, do you just make an appointment at a new doctor?

I LOVE my ped, but he is downsizing his practice by a lot and is away a lot. Plus I hate driving from Holbrook to Mt. Sinai.

Also, can you recommend a ped in the Holbrook area?

I just made an appointment at the new Dr. - told them I was looking to switch ped.'s.

At DS's first visit, they faxed over a release form to his old ped. for his records.
