Just go to SS and bring your marriage certificate and your drivers license. I don't know if you can do it thru the mail.

here is a link to the form you need to fill out...but yes you need to bring your drivers licesne with your new last name and ur marriage certificate to the ss office
ss form
go early so the line is not too bad
