I am going on a much needed vacation soon.
I am leaving my ds who is 21 months old in the trusted care of my moms, who always watches him when I worked, etc.

She was wondering if I should leave her with something in writing stating that god forbid anything happen to me, that she would be responsible in case of an emergency.
I don't like the thought of doing this. However, I guess its better to be safe..

I was just going to leave her with his insurance card and pediatrician number.

Have you ladies ever done what my mom thinks I should do?

Sometimes my mom can get a little carried away , lol

We, the undersigned, parents, hereby authorize grandparent or grandparent, paternal and maternal grandparents of cchild (SS#), to authorize any and all medical treatment for child they in their discretion see fit. This includes, but is not limited to, treatment to relieve pain.

A photocopy of this authorization shall be deemed effective as if it were an
original. This authorization shall remain in effect until date.

Just copy and paste into a word document, fill in the name sections and we get ours notarized.

Yes, I have done that when I left my son with my sister. I also recorded a video will. I know the thought of it is scary and may be uncomfortable. But anything can happen and I wanted to cover my basis.

My DH is uncomfortable with this stuff and says Im morbid and nuts. I just wanted my sister to be prepared as much as possible for worse case scenarios.

Yes, I absolutely would leave a notarized letter with your mom stating that she can make medical decisions for your child if need be. I would also give her name to the ped and authorize her to bring your LO to the doctor should she need to. It can't hurt, she's the child's guardian for a week......you should be prepared. Chat Icon

We do this for our students when we take an overnight field trip. Unfortunately an er visit happens most years (this makes us sound so reckless but skiing and skating with a bunch of city kids leads to a lot of injuries!) and the hospital is always so glad that we have a notarized authorization and copy of the insurance card.

I just looked this question up and this is what I found:

There is no legal requirement that the document is notarized. However, having it notarized helps confirm the signatures are actually those of the parents, which can't hurt in a medical emergency. Your bank probably has a Notary on staff that can attest to the signatures at no charge. If is preferred that you actually sign in the Notary's presence, but if you physically appear before the notary and state the signature is yours, it should be sufficient in this case.

OMG we are having the same issue. We don't have an official will yet (big procrastinators!), but we're going away without the girls in 2 weeks. I think we will just draft something quickly and have it notarized before we leave regarding guardianship. It makes me sick that we have to think like this. Chat Icon

I didn't even think of doing this! We're leaving DS for a little over a week in November. I was just planning on leaving the insurance cards and doctor info; but I will draft up the letter as suggested above. DH will think I'm crazy; but I'd rather have all our bases covered since we'll be away for so long.

Not sure it needs to be, but I think it should be. My ped wouldn't accept it otherwise.

Also, If you sign & notarize 2 forms you can leave one with your doc to keep on file, and the other your mom or caregiver can keep with her, just in case.

We went to St lucia for 4 days last year. We left both a medical form and got a brand new will done. Our Attorney said it should be notarized so someone in their office notarized it for us.
