question about NYS disability forms
This might be a dumb question, but I'm pretty clueless with all this stuff. Do you file before you go into labor, using the last day you're working as date of "incident", or do you file after? I got the forms in the mail, but it doesn't say. My FMLA paperwork is due 12/1, but it doesn't say anything on the NYS short-term disability forms.

you have to have everything filled out after you have the baby. you can't predict if you'll have a vaginal delivery or a section. the date of incident is the date of birth.

My HR guy told me to put the due date as the "date of incident". They can always update your information later.
