pre-admission forms
Did you mail this directly to the hospital? If so, what department? Admissions (sorry may be dumb ?) how many weeks were you when you sent it in?

No , my doctors office sent it in for me. They gave me the forms when I went for my first visit after my BFP, and I brought them back on my second visit. I am delivering at Winthrop.

I sent them in myself. My doc's office gave me a packet full of stuff that had some forms to fill out and an envelope ready to go. I don't remember where it was addressed to, though. Sorry!

I am delivering at Winthrop. My dr gave me the form but I had to go to L&D for a "false alarm" at 35 weeks before I gave it to her so I had to do it when I went to L&D.
You don't have to but it makes things go a bit faster. The last thing you want to do is filling out paper when you are in labor.

My doc gave me all the pre-registeration forms and I filled them out and mailed it to the address on the form. I don't know if it is right but I did it the same way for my DS and they had my forms there already when I arrived in labor.

My dr. gave me the forms (they ran out at our "tour") and I mailed it to the hospital...it had a pre-paid envelope so I didn't pay attention to where it went! Sorry.

I just got my packet yesterday and it gets mailed directly back to NSUH Manhasset or where you are delivering.

I procrastinated and wound up dropping it off at admitting 2 weeks before I delivered....np problems. I know there were also some forms that the dr had to send to the hospital as well
