I believe you need to be employeed and you would obtain the form from the disabilty company your employer uses. I received it with DS and since I handled the A/R and A/P I knew who to contact and called myself and printed out the forms I needed and mailed it in.

Our HR guy gave me forms to fill out and have filled out by the dr.

I called the NY Statutory Benefits Division and they told me to call for a paper claim form.

I get 6 weeks for standard delivery, big friggin deal. 26 weeks for medical issues. At $170/week, I could probably sell my breast milk for more. Chat Icon

I'm not sure if part time is eliglbe. I would get forms from both employers and see who they use then call up both disablity companies and see what they say.

$170 is the MAX. I guess if you make very little, it will be even less than that.

They say you get six weeks disability, but since one week is unpaid (the waiting period), You really only get paid for 5 weeks.

Yes, it depends on your company's policy...in my company, if you've worked under 5 years you get 66% of your regular salary and if you've worked over 5 years you get 85% of you regular salary, anything over 10 years you get paid 100%...it depends on the company.

See, I'm confused here also...my school is giving me my regular salary (I take mine 12 months) & I've been out of school since June 1 (but only b/c I had days)...they told me they are being compensated w/ the disability (but not sure if they'll get anything b/c my due date is July 2)...obviously NOT part of the school year.

DH thinks WE are entitled to that money, but I don't know...

I believe you're still entitled. Fill out the disability paperwork anyway the worst that can happen is you get denied.

ETA: You're entitled once you deliver (6wks vaginal, 8wks c/s) unless you have a medical reason to be out before.
