NSUH Manhasset
If you've delivered here in the last 3 years; how far along were you when you pre-registered?
I delivered here almost 11 years ago and I remember doing it early in my pregnancy, but now I'm almost 27 weeks and haven't received any paper work. Is this the norm?

My doctor gave me the preregistration package not NSUH. I think around 25-27 weeks. I remember it being around the glucose test because I was filling out the forms during the one hour wait.

Yes both times during the glucose test because I also filled out the forms during the hour wait

I haven't delivered here but I would ask your doctor as they probably have pre-registration packets. My doctor told me to fill it out at my last appt which was when I was 29 weeks.

My dr gave them me around my 24th week and I also received them from the hospital around my 34th week.
