licensed daycare and the Tax Form
DS went to another daycare earlier in the year but we took him out b/c we were having some problems with her. I have emailed her several times and asked her for the form. She emailed it to me but the document wouldn't open (from 3 different computers). She said she mailed one to me but I never got it. We are getting our taxes done tomorrow and I need the form!
Does anyone know if the form is a general form, or does it have specific information for the provider on it?

My guess is they really only need their tax ID #? You have their address and all.

I didn't have the form. I just used the tax id number and the address of the daycare and name of the daycare owner. And you need the amount you paid them.
