Now see, I'm going to say "have." I really can't get the gist of the entire sentence, but if you switch it around like this..."...operations has attempted to confirm..." it is not grammatically correct. So, "Operations have attempted..."

...at no point in time during the last six years have operations attempted to confirm...
the "in time in" doesn't read right.

The "has" should agree with "operations."
To make it clearer that operations is singular, I would refer to it as "the operations department"
"at no point during the last six years HAS the operations department attempted to confirm ...."
Even clearer would be "The operations department has not documented any attempts to confirm X during the last six years."

Yes, exactly what was in my head!
Forgive my ignorance though...I was trying to figure out if "operations" was singular or plural...never worked in an office!
