FMLA / NYC Dep't of Education Question
What doccumentation is needed for a childcare FMLA? I thought I had to take it to myOB but they will only fill it out for Maternity Disability and I am only entitled to 6-8 weeks.
I'm hoping to get 12 weeks off for childcare!
I tried calling my HR Dep't and got nowhere with them! I work for the NYC Dep't of Education. Has anyone else done FMLA through the DOE?

You can get the forms off the NYC DOE website, but honestly I dont know where. I have the form already but I will look at it and tell you what it is so you can look it up. I will be taking the remaining weeks of my FMLA when school starts up again. I will not return until Nov.
You Dr. will need to fill out a paper for your 6 or 8 weeks. Mine did. Then I sent it to my school secretaty. You will then need to get a second paper to take your FMLA.

Do you know which paper the doctor needs to fill out? Is it the pink carbon copy paper that is filled out for medical leaves, PD workshops, death in family, etc... (I don't remember the actaly name of the form).
Who fills out the FMLA form?

There is a workshop in November.
Here is the number for the Queens uft. They might be able to answer your questions about the paperwork.
