we used Fasco, dont know the #. I believe they are in the phonebook.

I used Fasco too, but didn't have a great experience with them.
As for pricing, it depends on how long and wide your driveway is, plus how much removal they will have to do - - plus, do you want block edging? There are a number of things that could effect your price. I'd get a few estimates to keep them honest.
