Dr. George in West Islip...
Anyone know the name of the office they merged with? I'm supposed to go to the Bayshore office for flu shots tomorrow but of course I lost the card.

The office where they sent us for the flu shot for DD is Dr. Louis Guida on Montauk Hwy in Bay Shore. HTH.
