Mine did in 2005 (bc/bs ppo) but I'm not sure about anymore.

United Healthcare and I don't think so. Although I honestly didn't inquire about it because we don't want one.

I don't think insurance covers if you are going for one that is optional. If your doctor has it in their office, and uses it, just as they would use the traditional, then I would think insurance covers it.

I dont think Oxford covers it either but like PP said we didnt inquire b/c we don't want one..
