Does anyone else have annovulatory bleeding ?

I'll try to make this as short as I possibly can Chat Icon Hoping someone can shed some light for me...

I have PCOS- Aside from when I was on BCP ( in my early twenties), my AF's only came 3-4 times a year...

When I started IF treatments, I was put on Provera to bring on AF and then given Clomid ( which that along w/ IUI is how I got my DS)...Then for my DD we did IVF...

After having my DD, my AF's ( well at least I thought thats what it was) started to come pretty much every month, except now often times I spot in between..

Whne I mentioned it to my OB/GYN he got very concerned, did some b/w and saw that I am not ovulating ( no shocker there), and that the bleeding I am having is not a true AF...

The way he described it to me, is that there is a series of events that takes place in order for a true AF to happen( basic biology 101for the most part)
What I am having is taking place, ultimately for no reason since there is no AF...

The reason he said it is dangerous is because, since there is no control of the lining ( I guess with the proper hormone levels that take place for AF to happen Chat Icon ), I could run into a complication and heaven forbid end up needing a transfusion one day ( he said he has had 2 patients in the last 6 months that this has happened to Chat Icon )

Anyhow, he wants me to take the Provera for 10 days and come back in 5 weeks for b/w to see if I ovulated...I never realized that Provera can make you ovulate ( being as I always followed it up with Clomid)...I was always under the impression it brings on AF and had no idea how/why......I also just started taking Metformin a few weeks ago ( hoping that helps)

Has anyone else ovulated on Provera ??

Sorry so long, I'm just so confused !!!

I have not had this experience, but just want to wish you luck. Maybe you can do some research about Provera and ovulation?

Sorry I can't help! Good luck and I will keep you in my prayers!!!!

I also have PCOS and have been on provera several times in the last few years. I've never ovulated (other than AF i got after 10 days i never got it again) when i saw the RE i needed to take i again in order to start clomid (which i did ovulate).

Re: Does anyone else have annovulatory bleeding ?

Oh Man Stacey, I wonder if this is what I am having. I've been somewhat regular since my AF came back but now I've noticed spotting in between.
I haven't had any BW done, but maybe I should make an appointment with my doctor.
I get really bad cramps and very heavy bleeding, would this happen if it was anovulatory, do you know?

ETA: Provera never made me ovulate. As far as I know it tricks your body into thinking you ovulated because it raises your progesterone levels, which would happen if you ovulated.

Re: Does anyone else have annovulatory bleeding ?

Stacey.. what your dr is telling you doesnt make sense to me.

Ive been to my OB and RE in the last couple of months SEVERAL times because I used to not ever get AF, and lately have been bleeding every other week.

Both of my drs said that it is not dangerous as long as there arent any other issues.

You can ABSOLUTELY get AF without ovulating and Ive had like 4 different drs tell me that. You shed your lining when you get a progesterone spike and then dont have pregnancy hormones to tell your body to keep the lining to support a baby.

USUALLY ovulation causes the spike, but your hormones are messed up due to PCOS. Provera does NOT make you ovulate. It tricks your body into thinking you ovulated so you get AF, which would then start a cycle and they test to see if you ovulate 21 days later by testing for the progesterone spike.

I am so confused as to why your dr is telling you this. a ton of women have ovulation problems and still get AF, especially women with PCOS.

My RE is not even all that concerned that I spotted for 7 weeks and now spot and/or bleed every 14 days.

I would cal and make a consultation with your RE and tell him your OB's concerns.

I just had a repeat water sono to rule out polyps...RE isnt sure what's up with the spotting so he is putting me back on metformin to see if it helps regulate me a little. He even told me he doubts very much that Ill ovulate even if my cycles regulate (I never ovulated on my own, even with clomid etc.)

I was rather confused myself, and as he explained it to me it made a lot of sense, but I am having a hard time reitterating what it is he conveyed to meChat Icon

According to him ( Dr.SanRoman) , a true period occurs because your body builds a lining to support an impending pregnancy after ovulation has occured..If pregnancy does not happen, that is when the lining will shed (which we all know)

He said some people ( due to hormonal reasons either related to PCOS or otherwise) will have an excess of estrogen ( or a few other things which my mind is escaping me at the moment) which will cause them to bleed ( and shed the lining that is there) however one of the big differences is that when a person is ovulating there are other things taking place like- the vessels in the uterine wall will contract back as part of the process...This is not something that will happen to someone who is bleeding for oither reasons aside from ovualtion...

He said many people like myself mistake these bleeding episodes for a period and they are not and it can be dangerous if not controlled..

I flat out asked him- (sorry TMI)

"Ok, I have blood coming from my va-jay
once a month, if its not a period what is this ??"

As for the Provera bringing on ovulation ( which I was like Chat Icon )

He said Provera (progesterone) "lightly "taps" the ovaries and it "can" make you ovulate, where as Clomid is like a "bang bang bang" to the ovary and injectibles are the big guns ( his words not mine Chat Icon )

I too only used to get AF ( bleed) 3-4 times a year, after having Alexa, its every month and I "thought" it was a good thing but apparently according to him, its notChat Icon

I also asked about polyps and he said he does not think thay is an issue for me...

He also said that left untreated ( meaning not regulating the bleed a few times a year with provera) puts you at a higher risk of developing uterine cancer ...

I am so confusedChat Icon , hence why I started this post ...

Lauren, I'm not sure, but I don't have heavy bleeding or bad cramps ( though I never have)...

Either way, its probably a good idea to get an appointment to see whats going on Chat Icon

Re: Does anyone else have annovulatory bleeding ?

Hmm Stacey. That seems to make sense, but of all the drs I have seen over the past 4 years has ever even brought this up.

I would be confused too.

I found this article online that seems to really explain things...

http://www.nursingcenter.com/library/JournalArticle.asp?Article_ID=534048

I am just wondering why my RE, who would specialize in hormone issues, isnt concerned? I am most definitely NOT ovulating and spotting/bleeding twice a month. Ugh. I just pray the metformin helps me!!!!

Wow- thats a great article !

It seems according to the "symptoms" I lie smack dab in the middle of both syndromes though Chat Icon

I don't bleed heavily or have cramping...I am wondering if the fact that I gained a ton of weight over the last few years ( and combined w/ my thyroid) if thats whats the culprit...

When I asked the dr. why suddenly after having Lexi, what would cause me to have an AF more frequently- the first thing he asked was what my weight has been doing...

You lost a bunch of weight right ? Maybe that was enough to throw you off too ??

As for the Dr.'s bringing it up....I had been under the care of another ob/gyn before I was pg w/ Lexi and switched to Dr.SanRoman ( who has a fantastic reputation and is the brother of my RE Chat Icon ) and he is the first to have any concern about this..I can honestly say he is definately NOT an alarmist type of doctor ( first hand knowledge after all my pregnancy complications w/ Lexi) , so I do have faith in what he is telling me, but I agree that its odd that none of your doctors have said anything of the likes to you ( maybe because your a little younger than me and not a risk factor ??)

Re: Does anyone else have annovulatory bleeding ?

Jen, that's a really interesting article. It sounds like I have AUB according to it, but the reasons don't really make sense for me. Although my last cycle was about a week longer than the rest have been - I think maybe PCOS is starting to rear her ugly head again.
Should I make an appointment with the RE or the regular ob/gyn?

Dr san roman is awesome. I used to see the female dr in his office and he is soooooo highly recommended by so many people (but he didnt take my ins). I dont know what the answer is, but I am popping my first pill tonight of metformin ugh.

Lauren.. I felt my obgyn, that I love, just didnt have enough experience to deal with my problems. He just wanted to stick me back on the pill and I wanted more in depth testing done before I start TTC again.
