Disability Questions
Does anyone know the website to download the form to apply for disability? My job is really small and has never done this before.
Also, if you plan on leaving but taking some vacation time so you get paid while out, do you have to apply for benefits once you have used up that vacation time, or apply the day you leave regardless ?

Also, you need apply for benefits once you become disablied. I requested my vacation time be used for that one week waiting period but my employer filled out the form from the day I officially left work. HTH

Who do you apply for benefits from? Your company?
My NY office is tiny, our hq are in CA they have a much better statewide disability/ maternity leave, so I'm clueless too!

so you didnt get your 1 week paid vacation?
So say I have 3 weeks paid vacation that I can use, I should have her apply once those 3 weeks are paid out? Even though I would have been out of work for 3 weeks already?
