Yes I believe you pay this if you refinance with a different bank.

We were with a different bank and had it removed (VERY annoying process) but had to pay $750 to get it removed!!!

I believe if you are going with a different bank, yes.
If you are staying with the same bank, I believe it is the difference between the original loan amount and new loan amount. For example if your current principle is $400k but you are rolling in closing costs and your new loan amount will be $415k, you pay the taxes on that $15k.
ETA: Talk to your bank about a CEMA.
