Yes - I had to when I had my back surgery.
Our company has a hotline to call to set-up the process.
I would contact your HR person first.

My company did it for me.
They kept whatever disability pays(which I hear is not much) and i was paid my full salary.

You and your doc fill out forms and your company will file for it... I think they paid about 5 or 6 weeks, it was less than $600, so lame!!

I did. My firm sent me a form. I filled it out and sent it back to them.
