BTDT moms!! Breast Feeding HELP!! need help for a member friend...

so my buddy just had a baby girl and she is having some breast feeding issues. The lactation nurses at the hospital were less than helpful.

The baby came 3 weeks early and her milk is not in yet. Both mommy and baby came home yesterday from the hospital. The baby was latching just fine in the beginning but then stopped latching and won't suck.

The nurses told my friend to just try and pump and see how that goes. They felt her boobs and it felt like milk should be in within 4 days. She tried pumping and there's no milk.

We don't know if the baby has received any or much milk yet since she didn't lose a bunch of weight at the hospital and has been pooping and peeing. She pooped 5 times between sat and sunday and twice today along with multiple pee's.

So now she doesn't know what to do. Should she buy 1 thing of formula for now until her milk is in (hopefully just a few days) or will that screw the baby up (in terms of confusion)? Is it possible there is milk when the baby sucks but not upon pumping yet?

she's not sure the reason the baby stopped sucking... she thinks there was milk in the beginning but can't be certain.

I'm clearly clueless on breast feeding so I couldn't offer much advice. I figured you pro's would hopefully have some answers!!

Please any advice would be greatly appreciated.... give as many details as possible. The 2 of us are clueless with this part!!

PS - a MAJOR part of the problem is that she won't suck and try. But she will suck on her mom's finger or a pacifier. What could cause no sucking? What could the mom do to stimulate the sucking on the boob?

If the baby is having poopie diapers and wet diapers then the baby is getting milk from the mother. Not all women respond to pumping, so you can't really go by that.

I am not really clear what you mean by stopped sucking. Does the baby begin to eat (suck) and then stops, and your friend doesn't know why the baby stops sucking? Or the baby will not latch on the breast at all. And if the baby will not latch at all, for how long has that been going on?

If she feels uncomfortable with waiting on nursing and wants to buy formula she can try the bait and switch. Have the mom give the bottle to the baby topless and close to the breast, then when the baby is in the middle of eating you take the bottle nipple out of her mouth and put the breast in.

She should also find a breast feeding consultant outside of the hospital ASAP.

Oh OK so it is possible she can get milk from the breast but not yet be able to pump! and the poop and peeing is good too then. that's great info!! thank you!

in regards to the 2nd paragraph you wrote about.. as I was told, the baby "gets on the boob correctly, but won't suck" she will suck her mom's finger or a pacifier but has stopped sucking her nipple since yesterday. She was doing it fine until last night or sometime yesterday and now she has stopped sucking and trying to suck all together. She puts her mouth there but doesn't actually suck. Does that make more sense? she doesn't start sucking anymore just kinda puts her mouth there and that's it.

that's what I said too. But since this just started happening last night she's trying to give it a whirl.
and especially since the baby was sucking at first and now just stopped she's more confused and doesn't know what to do to get her to suck again... if there's anything she can do she will do it. If not then i'm sure she will switch to formula.

tickle her chin, cheeks, side of mouth
wiggle her nipple in the mouth
bait and switch her pinky with the nipple
try a different position (baby sitting up always worked best for DS- like hugging a football to you)

my lactation consultant offered me this contraption that attaches a pouch of formula to something that attaches to the boob so the "milk" comes out easily.
i don't remember what it was called. (i didn't buy it from her)

maybe if the baby got the instant gratification of getting the milk right away w/ out having to work hard, it would again associate the boob w/ food and start nursing again.

Not really. You go by messy diapers. BUT if she goes to a lactation consultant they will weigh the baby before she eats and then after to see how much the baby is getting.

Is the baby crying a lot to think that she is not getting any milk?

Oh wow! I've actually seen something like this on TV. But it was a transgender couple who had a kid and the male was in the mist of a sex operation when the female partner got pregnant. Anyway the born male partner wanted to breast feed as He was going to be the mom and they has him use that device to get the baby used to breast feeding from him. They were giving him hormones to make his body produce milk. It was a wild story... but all in all I've seen that contraption before!!

where do you find a lactation consultant?

i've heard of the Le Leche organization... but their site seems somewhat confusing.

Not really. You go by messy diapers. BUT if she goes to a lactation consultant they will weigh the baby before she eats and then after to see how much the baby is getting.

she's not crying a lot. But she knows she's not getting any milk since sometime yesterday or last night because she's not sucking.

and since she tried to pump and no milk came in.. she started thinking perhaps her milk wasn't in yet.

she lives on Long Island... massapequa I believe it is. I'm so bad with town names sometimes. I know she sees dr's in the Great Neck area for certain though.

I had a similar problem with my DD. She was able to latch on in the hospital, but I was discharged first and I think she got used to the bottle. I would pump what I could (which was very little even though I was engorged), gave a bit of formula if needed and then tried to nurse when she wasn't frustrated. It took about 4 days at home trying to get her to latch on again...lots of skin to skin contact and lots of patience!

About the lactation consultant, I believe they are paid privately.

I always had to pay out of pocket, but I think some take insurance.

As for her milk supply, make sure she is getting lots of skin on skin contact, have her wear the baby. If she is comfortable with co-sleeping she can do that as well. High protein and high iron foods are helpful for supply, as is adequate hydration with decaffeinated beverages. Other things which limit supply are decongestants (pseudoephedrine & phenylephrine), brith control, parsley, sage, and peppermint.

There are lactation teas she can drink to help her, as well as fenugreek.

Kerri, I had the same problem with Emma, she was also 3 weeks early. I used a nipple shield to get her to latch, and that helped. Overall, she was just a lazy nurser..she would suck once or twice and fall asleep.

The tube thing is called a supplemental feeding system.

I can't edit from my iPhone..wanted to add that with both DC's, my milk did not come in until the end of day 4. If she's pumping now, she won't get much because it's just colostrum. Even when her milk does come in, she will not get alot in the beginning.

Kerri - I have a really good friend who is a LLL leader and is in Douglaston who could give her a call. BBM me if you want me to pass along her info.

I don't have a good story... as you know, DD starved and I never got milk despite everyone saying "just wait another way... wait another day... could be 10 days... could take 11 days... oh, if it's not in by day 14, you probably won't get it." But it's RARE and DD not only lost a LOT of weight but did not have good diapers until we supplemented. I used the SNS (supplemental nursing system) that medela makes to attach a little mini-formula IV to my boob to feed DD so she still would latch but got milk. It was extremely difficult to use but I did it for weeks waiting and waiting for my milk to come in.

If A's got good diapers, odds are that she's getting enough from the colostrum and it's ok. Have they been to the ped yet? We went on Day 4, 7, and 14. The ped and the LC at our ped's office would put DD on my breast and pour formula over her so she'd drink it off my breast as an alternative to the SNS. It's messy, but it worked.

You can FM or BBM me if there's any way I can help!

ETA: SNS on amazon -- pricey but I would imagine insurance would cover it through a company like edgepark considering everything. I got mine at the hospital.
