I think whoever your nurse is when you get in.
I made one with my wants and printed it out to keep in my suitcase. I'm just not sure if they really follow it or not.

I think its a great idea. Hopefully they will review it, it does seem like a lot of information but if we narrow it down it may be easier for them to read and follow. Good luck you are almost there!!!!!!
