Alimentum and Insurance Coverage
Does anyone know if Empire covers Alimentum, and if it does, how do I go about getting it covered?

Yes. I had it fully covered. Call the nurse line. You will need a note from your pediatrician saying it is medically necessary.
It was approved for 1 year. 5 cases delivered to my door every month. No charge, no copay.

Ours is covered. I called the pharmacy and the dr called in the prescription and then it gets delivered monthly.

Thanks for the information. I guess I'm confused because I can get the alimentum off the shelf at a pharmacy, say cvs. So do I need to order through insurance?

I don't have empire but usually you have to order it through a company like edgepark.

Yup! The formula will be delivered to your front door by a Catholic organization that also handles dialysis, wheelchairs, etc for patients.
ETA: if you need the contact info, let me know,

I would love the contact info. I'm completely overwhelmed by this whole thing (first time mom here). I see my ped Thursday for DS' 1 month check up and was told I can get the letter for the insurance company then. Does empire cover the ready made formula or just powder?

You can get whichever u want. Ds was on nutramigen and I got rtf. Now we are on neocate and it is only powder. Call your insurance company. If you have "durable medical equipment" .under your plan you will be covered. If you are, They will connect you with a company that provides durable medical equipment like edge park. The people at edge park contacted the pediatrician and I got 5 cases within a week.

Re: Alimentum and Insurance Coverage
Like the others have said empire does fully cover it. We had to pay $100 deductible and then the insurance covers the rest. We have the formula delivered to us through medical supply company Empire uses. Initially I just contacted insurance and they told us what we needed in order to get it covered.
Hope that helps!
