 Question for teachers who changed their name on their certifications after marriage... 
I never did this and I really need to.  Does anyone know the link on the state ed site or how I do this?  I was just searching around but got too many different search results.

I didn't change my name on my certificate. I figured that was me then, this is me now. I don't want to change it. Just like u wouldn't change your college diploma

www.nysed.gov I think.  There is a form that you can print out and have notarized.  I'll find the link.  I still have not changed my name in that area. http://www.op.nysed.gov/anchange.pdf

  Thanks for the link - that's actually the form that I found when I did the search - just wanted to make sure that was correct.  So they send you a new certificate?

I tried to change it but it never went through.  The website will tell you to send a letter stating your old last name and what you would like it changed to.  You also need to provide the information that is on the certificate.  I did this over a year ago and still no reply.  I am not going to worry about it because I only have my initial and I will be sending for my professional soon.

You have to mail them your certificate back.  I dont think you are allowed to hold two of the same. 
   Even though I go by my married name at work, I hypenated it on my certificate so it would match my BA and MSW.  
  Next month, I will start the process of the permanent.      Another $100!!!

I personally don't really care which name is on the cert - I'm just concerned if it will confuse things when I apply for perm cert though.  If I am applying for that in my married name, is that going to make things a problem since my current provisional cert is in my maiden name?
