2 active voting names/sites...how to get one deleted/inactive?

For some reason my maiden name with my parents address is still listing me as active. I haven't lived there is about 20 years and my license hasn't listed that address in anywhere from 15-17 years. I keep getting jury questionnaires every few years to my parents home in my maiden name/parents address. I have written in that I have been married 10 years and had three different licenses (even one in another state) but it never gets changed. I keep having to show proof.

I checked today my current polling place and my current info is correct and active. Out of curiousity I checked my maiden name and there it is. It says it is ACTIVE AND ACTIVE AS OF 5/2012.

I never get any jury info for my current name/address. They only send it in old name to my parents. Everytime they send it they ask me for the current info.

How does one remove this? I mean I would never do this...but someone who is not honest could vote two times since both are active.

Same for me - every year my sister says your name is still right above mine. I dont know how to get it fixed.
