When you interview for a position with another company do you usually tell your boss you are looking for a new job?
What about if you are applying for an internal promotion or transfer?

The company I work for does a lot of promoting from within. If you are applying for another position within the company they expect you inform your supervisor as a courtesy. Same applies if you are applying for a position with another company in our industry because we collaborate a lot with these companies.
I recently applied for a position with one of these companies and did not inform my boss for a few reasons. I suspect someone from this company inquired about me to one of my coworkers who informed my boss. Part of me feels it's only my business and my choice when and if to tell my boss and the other part of me feels guilty for not telling her.

Within the company, I never applied, but I know it couldn't be done without your manager knowing.
Outside the company, no one would tell and they weren't expected to tell. I'm sure it sometimes happens that it's found out in the same way it happened to you.

The last two companies that I have worked for require your supervisor to sign off on your application for an internal transfer. If I was looking outside the company I never told my current employer

Internal yes, you are expected to tell your supervisor, it's actually required and I have done it. Outside the company, no way in Hell!!!
