for those that did cord blood banking with viacord...
do you have to fill out that health questionairre? It's a millon pages long with a ton of questions. Is it necessary? I don't recall having to fill out anything like this with cbr?

We did CBR and will again this time, but I know dh had to fill out something online with a million questions. I think it's standard. You may just be thinking of the small index card that came with the CBR box.

I just used viacord and I had to fill the form. I also did 2 years ago for ds1.
