IVF Consult on Thursday- HELP
So I'm on IUI #6 (3 clomid, 2 Gonal F) this one is natural so my cysts go away. My insurance requires 6 IUIs before IVF. We are unexplained. So nervous it has come to this. Not nervous about the process- I can deal- nervous and worried about what if it doesn't work.
AF should be coming around August 12th and then on to IVF if everything looks good.
Any advice for my IVF consult? I'm going to write down a bunch of questions. Any questions you think I should definitely ask?
What are the typical meds used? Is there statistics about putting back one as opposed to two? Where can I find these stats? What are the different types of protocols? Where can I find information about embryo grading? Is i really up to you if you put back 1 or 2? When is ICSI needed? PGD?
Thanks for your input, I really have no idea what I am doing. I want to know what I'm talking about on Thursday- so much to learn. THANKS

IVF Consult on Thursday- HELP
This was my list:
IVF Consult Questions 1. How does monitoring work? How often will I need to come back for ultrasounds and bloodwork? What are the office hours, etc. 2. How will I find out my test results? 3. Can you give me information on what protocol you'd recommend in my case? 4. Fees not covered by infertility coverage 5. Do you require/recommend ICSI? 6. Would you recommend assisted hatching with the IVF/ICSI to help? 7. How many embryos would you recommend that I transfer at my age? 8. What are your current success rates for the treatment that you have recommended? (Remember, there is a difference between pregnancy rates and live birth rates) 9. Do you recommend any testing for either me or my partner? 10. How will I be able to communicate with you during my treatment process? 11. Will you be the only Dr. I see? 12. What protocol would I be on? What is a typical schedule for this protocol? 13. What medications will I be taking? What are any side effects of the protocol? 14. Does your clinic normally do a 3day or 5day transfer? For my age, how many embies would you recommend transferring back?
What are the typical meds used? A stim med be it gonal or follistim depends on what your insurance will cover. Some people will use menopur I didnt and ganirelex plus the trigger be it lupron or hcg.
Is there statistics about putting back one as opposed to two? Really depends on the embryos and your age. I only put 1 back during our fresh since it seemed to be the best at 5AA grade but it didnt work.
Where can I find these stats? What are the different types of protocols? Where can I find information about embryo grading? http://www.advancedfertility.com/blastocystimages.htm
Is i really up to you if you put back 1 or 2? It is up to you in the end.
When is ICSI needed? PGD? If you have mf or there is a fert. issue. Some clinics do it automatically with me we did natural fert. but when we do IVF again we will do ICSI since I had a 35% fert. rate. PGD is needed for testing of the embryos if There are lots of reasons why couples do PGD and one of them is genetic.

Re: IVF Consult on Thursday- HELP
I'm still researching stuff too . In addition to the questions and info I posted in my IVF journal and the articles I posted last week...here are a few videos and websites I found that go over the procedure more, explain some of the meds used, and explain the embryo grading. As far as how many embryos it depends on 3 things...your decision, what your RE is willing to do, and how the embryo grades.
You can find some stats on the SART website, however, I've learned through my research that they are not very accurate. They are at least 2 years old and they are not objective. REs self report their data, and can embellish or omit data if they choose.
When they evaluate the egg after 24 hours, if fertilization has not occurred, ICSI is used.
If there is a history of multiple losses or a history of genetic disorders, or if the patient requests, PGD can be performed. This is a great video! http://www.youtube.com/watch?v=kcJWPWb2uBs

Thanks so much for the info. Stealing your questions! I know I will be on Gonal bc thats what I used for IUIs. I only have 2 pens left and I don't think that will be enough though. So in addition to Gonal I will probably use Menopour or Garnilex? As far as age I'm 32 (birthday was Sunday). My RE seems to be very conservative so I coubt he will suggest putting back more than 1. At this point whatever ups my chances.... Do you think its a significant percent higher that 1 will stick if I put 1 as opposed to 2? We are unexplained so it will be interesting to see if RE suggest ICSI. I hear thats like 2k.... Thanks again and good luck.

Thanks so much I will definitely check out those websites. So with ICSI we can choose to do it if needed it doesnt have to be decided ahead of time? There could be a fertilization issue since I have never been preg....who knows. Stupid unexplained. Thanks again...I will be stalking your IVF journal so keep it coming. I'll share with you anything I learn on Thursday.

IVF Consult on Thursday- HELP
Yes some will include menopur. Ganirelex is used to prevent ovulation while your eggs mature. I am 32 and we did 1 ivf where we transfereed one (bfn) and a fet where we transferred 2 and got pregnant with our daughter. Putting 2 back increases your chances of one or both sticking it depends you just never know if they are normal.
Also if you decide to not do icsi and there is poor fert they can do rescue icsi. You won't be able to know until the egg and sperm are put together.
Good luck

How long were you on Garnilex and how much of it did you have to use? That is great you got preg from a FET. I feel like if it ups my chances by evebn 5 percent I would want to put back 2. My RE is more conservative and has mentioned putting back 1 for my age. But what if that 1 isn't normal? Uhhh a lot to think about. Did your RE say how much it would up your chances putting back 2 instead of 1 on your FET? Do you have any frosties left? I'm gonna see what my RE recommends with ICSI. If it increases our chances by a good amount then I'll prob do it. I mean at this point whats spending more money....ha. Thanks again! I'm sure I ill be posting lots of questions.

IVF Consult on Thursday- HELP
I stimmed for only 8 days and was on ganirelex for 3 of them. It really depends on the quality of the embryos more so than the age. I was 31 when I did my first IVF and we were given the choice to put 1 or 2 we decided on 1 since it was a great hatching embryo but it didnt work out.
I forget what the increase was when putting 2 back in but for a FET they recommended 2 anyway and I have only gotten pregnant with singletons on both FET's.
I just used my last 2 frosties. I had 5 embryos when my IVF cycle was done.

Thanks again for your help. 8 days seems pretty quick to stim. I bet I will longer bc on Gonal for IUIs I do like 9 days. Is Garnilex similar to Gonal F or it does something different? Does it come in a pen? I only have experience with Gonal F pen. I want to make sure I order enough of whatever meds so I have it incase I need to do another IVF. Congrats again. I'll be asking lots of questions tomorrow when meeting with my RE.

Re: IVF Consult on Thursday- HELP
I just got the big box of meds:
In it was: follostim, menopour, novarel, crinone, and gararinflex. I'll also be on estrace.
right now I'm starting BCP so they can "map out the dates" and I call tomorrow to see when I'll start stimming.
I get the impression that my ER/ET won't be until end of Aug-beginning of Sept.

I saw your other post first. Wow thats a lot of meds. I know I'll be using Gonal F since I was on that already for IUI instead of Follistim. Novarel is the trigger. Not sure what Menopour and garinlex are and when they are used in the cycle. Not sure what Estrace does either. GUess I will learn this tomorrow.
Are you on Birth Control in order to sync your cycle with other ladies? I never had any side effects on BC. How long are you actually going to be doing stims? Let me know what they say tomorrow and I'll let you know anything new I hear from Dr. Z tomorrow.
