hospital pre-registration form
is it really only one page with mostly-basic info on it? i feel like my doctor didn't give me the right thing 
i'll be delivering at LIJ Manhasset

i'm not delivering there, but i was able to find my hospital's pre-reg form online and it's affiliated with NS/LIJ. (I'm delivering at Lenox Hill).
So, maybe check the website?

Mine was double sized- basic info (our names, address, bdays, ssn, race, religion, etc). The opposite side was a questionnaire like if I drank or smoked during pregnancy.
I also had to sign an authorization form.

I am delivering @ LIJ-NHP - and yes, it's just that simple.
When you get there to have the baby, they will get more info from you when they register you.

Yup one little page. I don't know where I put mine... I thought I'd filled it out but now I'm pretty sure I haven't mailed it. Gotta pull that thing out and put it in the mail.

Mine was just a few simple questions like name, address, insurance info, and spouse's info. I really thought they would need more information, but I guess that's it! I'm delivering at St. Charles, btw.
