Re: fireplace?
well, it really is variable. We just did it as part of our whole house renovation. I don't know how much it really cost since the plumbing for the gas was in a large plumbing quote. we did the framing of the bump out ourselves, had the fireplace store do the fireplace install, and then had a seperate mason do the stonework for the harth, fireplace stone to the ceiling and put a stone mantle in.
I'd venture a guess that it in total probably cost 8-10K. However, we bought a nicer 40 inch fireplace, did real stone veneer from floor to ceiling with a harth. It kind of got a bit out of control.

Re: fireplace?
Are you planning to use your existing chimney or will you need a new chimney installed?

We are planing on doing this as well. Do you have any pictures? Also which venders did you use and, would you reccommend them?
TIA

Re: fireplace?
Hi, I don't have a pic online, but it looks similar to http://www.mjmmasonry.com/projectgallery.php?projectid=27
basically the same stone, to the ceiling. my mantle is actually a piece of brownstone, and I also have a raised hearth that is topped in brownstone.
I bought the fireplace at Embers in Farmingdale. They are pretty good, price was decent. they did the install and venting of teh unit. my plumber did the gas piping, I wouldn't recommend him.
the face of the wall was covered by JP Miller Concrete from Wantagh. He did the ledgestone and brownstone.
the framing on the bumpout we did with my brother in law.
I think Embers can do almost all of this, including contracting the plumbing, framing etc. but not sure.

Re: fireplace?
We bought the fireplace and stone and hired separate companies. Our contractor doing our master bedroom Reno framed it and we hired a plumber to run the gas line. DH put the cement board up and is working on cleaning the grout off as I type :). We're making a mantle out of the wood steps they sell out lowes. We had bought a corner mantle and then decided on a different location. I would say total it cost under 2500. When we had gotten quotes from the fireplace store just for the unit and mantle it was 4000, maybe more, it was awhile ago. We go our unit from direct buy and saved a ton. Ours is 24" 99.9% effiicient. I have a nice white corner mantle if anyone wants it they can take it.
