Quick Question
My older son is going away this weekend with my parents. What should I put in the "in case of an emergency note?" And does this have to be notorized?

I have no experience with this but I would just include dr's numbers/names, what he may be allergic to, where you can be reached, if you can't be reached someone else to contact, your address.

I am pretty sure the letter needs to be notarized. My parents always got the letter notarized when we were younger just incase. It wouldn't hurt to have it notarized even if it's not required. You never know.

Make sure that you include a note, signed by you, that states their names and that you give them permission to make medical decisions for him. My mom takes my kids to the ped. frequently, and they still check their records every time to make sure that they have that note. It does not have to be notarized as far as I know.

My mom has written notes for us before and they weren't notarized but I don't think it will hurt if you get it done, just in case.
