Definitely October. It's beautiful that time of year. Only south of Rome, definitely Naples and Pompeii!
For first timers, I definitely recommend several days in Florence and no more than 2 nights in Venice.

In may and like practically everyone said rome florence venice..the big three. My absolute fave is like someone mentioned...montepulciano. omg so amazing.

I also wouldn't recommend, as much as I love it, Milan on a first trip. it's not really the Italy people expect, as awesome as it is.
I think I would have to agree - of all the places we saw, Milan and Naples were the most forgettable in comparison to Rome, Florence and Venice.
I truly don't care where we go, I loved it all, and this trip is really more about DH seeing it for the first time, but I figured we'd so Rome and south because that's where his family is from. I may have to re-think this!

if it's his first time, ask him. if he has ancestral roots there, I think it would be awesome to take him and Luca and Izzie to see where there greatgranparents grew up!!!!!!!
I don't have ancestral relatives there...I have transplants so Milan it is for me. but I think if you have a link to someplace, you follow the road to that link before you go see what the tourists do.
ETA: you cut me deep though. Milan. forgettable? FORZA FORZA MILANO!!!

DH has been several times and has family there. His father was born there...
He said, by far, the best time to go weather-wise is Fall. September or October. He said the second best, in his opinion, is Spring - May. He said that the summer gets VERY hot and the winters are really rough.
He said, aside from Rome like you mentioned, he loved Venice, Florence, Tuscany, and he loved Capri and Sorrento. Then there's landmarks - you go to Pisa to see Pisa, you go to Naples to see Pompei. Then there's tons to see in Sicily.
DH said Italy is tough - there are SO many beautiful places and you could go for weeks and not see everything!
He suggests to everyone to definitely stay in a small town once while there - everyone always does the big cities - he said that going to a small town/city is a different experience and you really get a sense of the hospitality and culture. It's amazing.
I've never been YET - I asked DH for you. He's been 89475897345 times lol.
