YOu have to apply at the Post Office. Go to CVS or Eckerd's and get passport pictures done, then go to the post office and fill out the paperwork. I remember needing mone orders (like one for the passport, one for the post office i think), but depending on who the passport is for, it's different prices. YOu also need to bring your original birth certificate, which they'll take from you and mail with your application. You get it back when the passport comes in the mail.
That's how I did it!

also if it is for the kids both parents need to be there when the paperwork is handed in at the post office.

check on the USPS website they will tell you the closest post office near you!
Costco has the cheapest pics then take them to the post office...
and good tip from PP about the kids passport needing both parents..I learned that the hard way and had to go back with DH =(

Travel Dept website
Just go there and click on passports and it can take you thru the step by step process -- including allowing you to fill out the passport application on line, and then it creates a printable PDF for you to print and take with you to the post office.
The fees and information are all there for you.
