Putting spouses name on house
How do you go about it? Do you have to re-fi?

DO we go to the county courthouse for that or the bank? Sorry, I have no idea.

If you want to do it yourself, call up the assessors office at your town, they should be able to mail you the forms or you can pick up. You have to go to Riverhead to submit the forms or you can mail them in but it takes longer. I think the fee was $98. ETA: oops, sorry. This is for suffolk county only.
My attorney prepared all the documents for me so it was very easy. While I was there, I also had my last name changed on the deed. If I remember correctly, my lawyer charged around $200. Let me know if you want her info. She does both nassau and suffolk.
