Can't tell you how many times I got bills that were paid. They probably need to resubmit the bill or it was already taken care of. Can you get onto your insurance website and look up claims. Plus if the lab missed the deadline to submit the claim, that is not your responsibility.
Its all very confusing. I still get billed for stuff that was paid a year ago.

Your insurance probably has participating providers that they cover. For instance mine doesn't cover Quest Diagnostics so if I go there, I have to pay for it.

My insurance didnt cover any infertility treatment whatsoever. no bloodwork nothing. I called enzo labs and they gave me a discount and a payment plan..It was like 50% off.. So if they do not cover this call them speak to a supervisor tell them that you can not afford this and ask if they can do this for you.. and 50% is probably more than the insurance pays anyway.. thats why they probably do this..

This is how my insurance is (but opposite). If I go to quest I pay nothing out of pocket and no co-pay if I go to their blood drawing center.
