Does anyone have Empire Prism EPO?
I feel like crying. In fact I am tearing up right now. I just found out that effective 12/1/08, my company is switching insurance to Empire/Blue Cross Prism. I have to say goodbye to Dr. Flisser and I am heartbroken that I need to find a new doctor.
I don't even know what my benefits are yet. Does anyone have this plan? I have a bad feeling that my coverage is going to decrease.

I am so sorry! Call them ASAP and have them send you a printout of your IF benefits!
Even someone with the same plan as you might not have the same coverage as it is dependant on your own company's policy.

Yes me, well I have Empire Prism but a PPO. The following is what they list under IF Coverage. They define infertility as the inability for a couple of achieve pregnancy after 12 months of unprotected intercourse
The following is covered: IUI D & C - including any inpatient or outpatient hospital care that would correct malformation, disease or dysfunction resulting in infertility. HSG Hysteroscopy Endometrila Biopsy Sono Hysterogram Post Coital tests Testis biopsy Semen analysis Blood tests Ultrasounds & any other medically necessary diagnostic tests and procedures, unless excluded by law.
Not covered: IVF GIFT ZIFT Reversal of elective sterizations Cloning Experimental medical or surgical procedures Services to diagnose or treat infertility if we determine, in our sole judgment that the service was not medically necessry sex change procedures
Unsure quite why they list that last one with IF coverage & services.
As a pp said each plan is slightly different so I would check. Also some drugs are covered, some not you would need to see what drug coverage you will get.
