When you submitted you were going out on maternity leave
Did you do something in writing to be given to HR and your manager stating effective such and such date I will be going out on maternity leave? or did you just tell them ok this date is my last date?
I am thinking to cover all my bases (especially with this place) to do something typed up stating effective this date I will officially be going out on Maternity leave to hand in to my manager and HR.
Just not sure what to write up. Any help would greatly be appreciated

Your HR dept should have some FMLA paperwork. Check with them.

I have all my FMLA paperwork. Handing it in next week. I wasnt sure if I had to also submit an additional letter or something.

my HR dept asked me to... basically my doctor wrote a note on a script pad that said i was due 9/8/08 and was under her care. that's it!
i would check with your HR but i don't think it's totally necessary

Thanks. I dont think it's totally necessary and the HR gal see's me everyday so it's not like she doesnt know 
Just wasnt sure if anything else had to be submitted with the FMLA paperwork.

I got FMLA paperwork taht asks for an estimated date that the leave will start. I plan to work til the end, so I do not have a definante to put down.

All I have to do is have my insurance papers filled in and call HR when she is born. I must also make sure I pay the money to maintain my health insurance (which is a nominal fee) while I am on leave. Then my doctor submits disability. We don't have any other forms to fill out. Thats it.

my co-worker still had to fill out the FMLA paperwork, i think... but she ended up using just accrued time for as long as she wanted to stay out.
not sure what your policy is, i work for the county  i wish i had enough time to go out paid!!

I went to my HR dept and they had a whole packet of info for me to fill out including all the FMLA info
