Had my consult last night
Met with Dr. Brenner. After he reviewed my history, including the results of my HSG, he has advised me that he won't pursue IVF with me until my left tube is removed and what remains of the right one is cauterized. He said the risk of ectopic from IVF in someone with my history would be too great to chance. Plus, the fact that my left tube was diagnosed with hydrosalpinx (sp?) adds a greater risk of miscarriage as fluid may be leaking back into the uterus. So, I need to get back with my OB/GYN and schedule surgery first. I made an appointment to see him next week.
In the meantime, he does think that once that's solved we are great candidates for IVF. He thinks we can start the month after the surgery so he's targeting an April date if I can get the surgery done soon. My exam was good - he did see me ovulating on the right side. Both DH and I had all our blood drawn for tests and they recommended that we get the SA done, as well as the IVF interview and the social worker visit so that after the surgery, we're ready to go.
Yet another hurdle to get over...although I do feel better that we have a plan, and I will be happy to get these tubes out so I don't have to worry about ectopics any more. I kind of had a feeling this is what would happen, and I'm not looking foward to more surgery, but I'm trying to stay positive now that it will all be worth it in the end!

We had to do that at my first clinic

We had to go to one. We went to the SW in Port Jefferson. Cannot remember her name.

As Lisa said above, it's required by the clinic. Sounds to me like it's kind of like "pre-cana" for IVF'ers. They want to make sure that DH and I are on the same page, that we understand how difficult/stressful/emotional this process is, as well as if we are on the same page on the larger moral/ethical issues. For example, the nurse told me that they may ask us what our plan for any frozen embryos would be if we were to get divorced. That kind of thing. It's certainly nothing specific to us! 
The plan is moving along, btw - I meet with my OB/GYN/Surgeon on Thursday to plan for the surgery.
