if I send an outlook mtg request to someone at another company...
And they accept it... will I get an acceptance email?
If I send one to my colleagues, I get one when they accept. However, I sent one to DH and go nothing in return. No idea if he accepted it or not.
If he does accept it, should I get an email saying he accepted?

Even in your own company, you aren't guaranteed a response. If the other person doesn't send the response, you won't get it. Sometimes, if a meeting gets changed often, or if I forgot to accept, but went to the meeting anyway, I'll accept it after the fact, and not send the response.

Look at the appt on your calendar and look at attendees - if it's tentative for them, then they didn't accept (I think!).

Tentative means they responded as tentative (it's an option). In your own office, you should see who is attending, even if they didn't send a reply. In the Tracking tab (at least in Office 2003), it shows the response of everyone listed.
Since people outside of your office might not be able to respond back to your Exchange Server, I'm not sure if it will show for them.....

That might be a setting.... In my calendar, it shows "None" in the Response column if they don't respond. Accepted or Tentative if they did respond. I'm not saying you're wrong, just saying what I see in my calendar. There may be ways of having defaults, or if you are a delegate for someone, the display may be different.
