Dear Coworker:
Did I ask your opinion about my feet and ankles today? YES, I know they are enormous and YES, I watch my intake of salt ritually, even before I was pregnant. NO, I do not need your stupid comments in front of the entire staff AND patients!!
Please keep your opinionated, snobby, loud-mouth comments to yourself!

I know it's hard (because I'm the worst at this), but I really think you need to put her in her place. Even nicely. I would just say something like, "Listen, I appreciate your concern, but your comments are really hurting my feelings. I know you're trying to help, but I'm fine! Really!"

 i get this all the time too. Mine have been swollen for 2 weeks straight now - - i wonder why people are so mesmerized by them??? ask her if she wants to rub them for you
