HR people - interview assessment forms
I've been asked by my manager to take over the interview process for some new positions. At a previous job of mine we had interview assessment forms that we used to rate candidates on a scale of 1 to 5 for each point. It discussed general feelings about the candidate, knowledge, etc.
I was wondering if any of the HR people had anything like this that I could see? I have looked online and found a couple but nothing that really fits.
If you don't have a form like this - could you offer some suggestions as to what I should look for in a candidate so I could make my own form. This is for a technology job, so I obviously would have technology questions and could rate them there, but I'm afraid I will miss something more 'general' about a candidate.
I haven't interviewed anyone in over a year so I'm rusty!

I also Always ask what do you like most/least about your current position... will give you an idea of the "fit" for your position....

I have a 5 point scale on my interview forms, but it's company and positon specific. Maybe you can work on coming up with the top 5 attributes you want candidates to have and start from there and make your own rubric.
ETA: This is a general sketch of what my rubric would look like converted to technology
Is an expert in the job-required technology
1-5
Will represent the company in a professional manner (and you can be more specific about this in terms of your corp culture) 1-5
Has relevent experience
1-5
Show dedication and commitment to this job as a career
1-5
