It is OK to put in your social, they use that to verify that way random people can't claim your money. It is a state-run website so it should be safe.
As far as the address - put where you want it to be mailed to as your current address and then the past address you have to provide some kind of proof, usually, that you lived at that address. This is to, again, make sure random people don't try and claim money that is owed to you.
For my dad, which is what I originally posted about, we haven't lived in East Meadow since the mid 1980's! So, he had to dig up an old mortgage document with his name and the East Meadow addy on it. I made a copy and snail-mailed it to them using the instructions on their website.
With my mom, it was a payroll check that was owed to her and it was from when she was at her current addy so she didn't need to send anything in and the claim was filed online and came in the mail, 1-2-3.
With my dad, because of the address change it was a little bit more work.
So, you could either call or put in your social and see if it prompts you for proof like it did with me in my dad's case... if it does, just follow what it tells you to do. I had to print a document and get it notarized!! That was part of the process with sending in the proof that my dad used to live in East Meadow.

Yeah if it has your old addy on it... Anything that's proof you lived there. Your name and the address on something works.
You have to print the form they provide you with, make a copy of your proof (so, your DL) and mail to the addy they provide in Albany -- but get it notarized :)

to the people who have gotten substantial amounts of money - how does this money go missing in the first place? when you see the claim and the amount do you have an "ah-ha" moment and remember something you missed years ago or is it a total mystery? i don't get how a store like bed bath can owe you money and it doesn't get refunded to your credit card.

I told my BIL about this and they got back over $1000 from an old job. Now how he walked away from a job not knowing he was owed that type of money is another story!

I don't know about the store but with my mother, it was a payroll check that she just never got. I don't know if she never cashed one by mistake and then it goes into her "account" in the company and cut to ten years later and she's no longer there and somehow it makes it to this database?? Or is it alllllll kind of tracked by your SS# and that's how they know she is owed this?
It's kind of amazing -- I mean, it's been so many years since she worked there and she got this check. And she didn't have a light-bulb moment of OH YEAH, I forgot to cash this! It was a million years ago and who keeps track? She was like, oh wow, money LOL!

That's great -- I mean, to me, I'm thinking was that little bits of money over time that gets taken out of his check and put into some kind of IRA or 401K or 401-something-or-other that these places sometimes set up for people and we don't even notice the money going in there each time??
And then you end up leaving for another job and don't always think about that, or it slips through the cracks. I could see THAT happening, maybe.

Hmm that would be something I would call them about. Could it be another person who has the same name as you or you KNOW it's you but the address is a typo or an error?
Yeah and I would call the number and ask either way.

The description says amount due for undelivered goods and services... I agree though it's weird - especially since they have my address - couldn't they have just mailed it to me?

when i called there was no human to talk to. does anyone know how i can reach an actual person?

I always check too.
Nothing for my married or maiden name.
Nothing for DH.
Not even 3 cents!
But I guess that makes sense- I don't' recall anyone ever owing me money.

I referred a friend to do this after I searched for her name and she got a $1,400 check from prior employer and said the same thing. She was thinking maybe they had paid out her vacation wrong when she left but she couldn't believe it either. I ended up getting $66 from BB&B, no idea why I was owed that.
