UPDATE so..
we got our contract back fully signed yesterday and the bank appraisal is scheduled for tomorrow AM. 
Hopefully the rest of this process will go smoothly now

We had the inspection for the appraisal and we missed the fact that there was no smoke alarm in the basement or on the 2nd floor 
Have to fix that and have the bank come back..

did you have an inspection before contract? they missed that?

Yes, we had our engineer come in, but it wasn't a "real" inspection. He is a close family friend who did us a favor. There were no major issues with the foundation, etc. which is what we were really looking for at that point..
really silly in hind sight, but nothing we can do now
