Power of attorney healthcare

Dh and I are going to Paris for a week. MIL is watching dd's and asked that I set up a power of attorney healthcare in case of emergency.
Is anyone familiar with this? I am not sure if a letter with my signature is enough or if I need something notarized. Any info would be helpful.

Re: Power of attorney healthcare

I don't know much about is since I've never really left my kids for more than a day or too but given that you will be away for a week, I think it is a great idea.

I think what they are talking about is a a Medical Consent form, which gives someone else the authority to make medical decisions for your child while they are temporary away from you.

A power of attorney/healthcare proxy is something you set up in case YOU become incapable of making your own medical decision and appoint someone else to make them for you. (so, I could appt DH or my parents my healthcare proxy if something where to happen to me.)

Ask your pediatrician's office. I know if you call and give them the name of someone who watches your kids, it is enough for them.
