Peditricians office and forms....
Does yours charge to fill out forms, such as daycare forms or camp forms?

No but a lot of offices do now. Some charge a yearly flat fee, some charge per request, it all depends.

My ped's office has yet to charge me for forms....
I have an appointment next week for a camp physical. Last time they just filled out her height and weight on the form...attached a copy of her immunizations...and that was it...part of me doesnt mind paying if they have to really fill something out or write a letter for insurance..but to just sign the form and attach a copy of immunization records would kind of annoy me...

I haven't used it but I think mine has a sign that says it is $5 to fill out forms.

Mine charged me $25 per child and I'm really annoyed with them. They are nickel and diming for everything and it's becoming very frustrating.
I just got a bill for 'Surgery" which was basically the Doctor removing some wax from DS's ear because he was getting his ears checked and had an ear infection.
