for me it was the crib vibrator. Made the transition from bassinett to crib a snap, and had the baby sleeping through the night really fast. Sound activated, so if he cried, vibrator came on, and soothed him back to sleep.
Of course, this all depends if your baby likes vibration or not, you do not need this item until later on when you will be able to see if you baby likes vibration.

I didn't register for a video monitor, but a friend gave me hers... I LOVE IT! It seemed a little extravagant to me, but I have come to love it because I can figure out if it is time to go into the baby's room, or if he is just making noises in his sleep.
I also could not believe how many burp cloths I used in an average day. Now, the spit up/dribble is not as bad, but is drooling from teething. I am constantly wiping him down.
we have also used the swing a lot. DS is napping in it, right now!
The snap and go is also awesome. I use it almost every day.
The pack and play has also been a lifesaver. DS does not usually nap in it anymore, but I still keep it set up downstairs and use the changing table part several times a day.

it has only been a month but so far our must haves...
boppy miracle blanket/swaddle me cloth diapers for burp cloths moses basket body positioner (snuzzler) tiny love mobile aquaphor glider chair dr. brown's bottles newborn sized clothes (dd is small) just now she is getting big enough to try to bjorn, but we hated the sling
