When my BFF turned 40 last year, I bought her a very nice watch with a card that said "Now you can turn back time". Kind of corny, but she loved it.

My cousin with 4 kids just turned 40. Some of the things I remember her opening and loving were:
Michael Kors bag
New Makeup
Coach Boots
New Robe and nice pajamas
Tickets to Broadway Show

Plastic surgery to bring the floppies back to their original positions on my chest. that's what i want for my 40th.
But seriously, I'd want something I can do for myself..
A Spa day with babysitting services by you
Massages
Mani/Pedicures
Tickets to a Broadway show complete with free babysitting for the night
ETA: a friend of mine turned 40 last year and her friends threw her a "toy" party. They rented a room, hired a dj and everyone wore lingerie on teh outside. It was a blast Almost like a bachlorette type party.

I would get her something really girly and fun—something that she'd never splurge to get herself. Depending on your budget, maybe a designer bag or a bracelet.
