I once found a screw in my meal at Cucina de Pesce in Manhattan. Not as disgusting as a bug by any means but bizarre as hell. When then manager came out, all he said was sorry, we were fixing a pot handle and it must have fallen in. Didnt even offer to comp the meal. 
Years ago my brother found a fingernail in a Filet o Fish at McDonalds. Complete with red nail polish

Paper in DH's food at Applebees twice, 2 different applebees. Needless to say..
