I'd go to the game, miss the church, and meet them all at the party. You have children who have activities too, you're making spots for both of those things on the same day! I wouldn't feel guilty about it at all!

I voted but wanted to respond here...
Go to the game...it's a big one (and important one) for your son. He comes first. Don't even worry about what they think. Can your DH go to the church to 'represent'?

I am an avid attendee of the ceremony the party is for. So I would probably get to the church late. BUT- if it's going to make you run ragged, skip it. But let them know in advance.

I too believe in attending the actual event in addition to the party. I would be late to the church so that your DS can be in his last game. I don't think there is anybody who wouldn't understand -- especially the ones who went to the make up game.
